Designated Driving Services is not a taxi service!
We are to be used in emergency situations only!

I recognize as a young teen that there are many potentially dangerous decisions I may encounter everyday but I will do everything in my power to avoid making the wrong decision that will jeopardize my health, my safety and your trust in me. I understand the dangers associated with the use of alcohol, drugs, reckless driving and the destructive behaviors often associated with impairment and reckless driving. I will read and discuss each rule of this contract with my family.

1. Teen Agrees To the following "no alcohol" rules:
___ I will not stay at parties where alcohol is being served.
___ I will not ride in a car with a driver who has been drinking.
___ I will call a parent/guardian or use my DDS card for a ride home if my driver is impaired.
___ I will call a parent/guardian or DDS for a ride home if my date is making me uncomfortable.
___ I will not allow my friends to bring alcohol into my house or onto my property.
___ I will not be succumb to peer pressure by drinking alcohol.

I have read and discussed each alcohol rule above with my family members. By signing below, I promise my family and DDS to honor all the rules. If I break any of these alcohol rules, I will accept the consequences, which are: (______________________________________________

2. Teen Agrees To the following driving safe rules:
___ I will not use a cell phone while driving
___ I will not send or read text messages while driving
___ I will not play loud music while driving
___ I will not eat or drink while driving.
___ I will wear my seatbelt at all times
___ I will make sure my passengers wear their seatbelts at all times in the vehicle
___ I will always obey the posted speed limit

I have read and discussed each driving safe rule above with my Parents/Guardians. By signing below, I promise my Parents/Guardians to honor all the rules. If I break any of these driving safe rules, I will accept the consequences which are:(______________________________________

By signing this contract below, I promise to abide by all of the rules mentioned. I agree to communicate with my parents/guardians regularly about issues of importance to both of us. All discussions will promote mutual respect. Even if we don't agree, we will be respectful and not raise our voices when speaking. We will keep communication channels open at all times.

I recognize as a Parent/Guardian that there are going to be many potentially dangerous situations and decisions that my family and I may encounter at anytime. So, I will do whatever it takes to make sure that my family and I avoid making decisions that will jeopardize their welfare and their trust in me. I understand the dangers associated with alcohol, drugs, reckless driving and the destructive behaviors that are often associated with it. I will read and discuss the rules of this contract with my family.

1. Parent/Guardian Agrees To the following underage drinking rules:
___ DDS or I will pick up my teen any time he or she calls for a ride home.
___ DDS or I will pick up a teen who refuses to ride with someone that is intoxicated.
___ I will not make alcohol available to anyone under age 21 at my home.
___ DDS or I will provide transportation home for you when you cannot get home safely.
___ If any of these situation should occur I will not discuss it until the following morning if need to be.

2. Parent/Guardian Agrees To the following drinking & driving rules:
___ I will not allow anyone to leave my home intoxicated. I will call DDS to take then home.
___ I will call DDS to pick up a family member that is out and has had too much to drink.
___ I will call DDS or a family member for a ride home if I have had too much to drink.

I have read and discussed each underage drinking rule above with my family members. By signing below, I promise my family and DDS that I will honor all the rules.

By signing both contracts, all parties are agreeing that if they find themselves in one of the dangerous situations stated in either contract, they will call Designated Driving Services, a cab or a family member for a ride home with No Questions Asked and the situations Will Not be discuss until the next morning in a calm and caring manner the next morning if need to be.
What's important is that you may have just saved a family member life and better yet save an innocent persons life on the road.
